Please enter your email address and the code provided by your school. Then press 'Find School'. It is important that you enter the correct email address as future correspondence will be emailed to you about your child's vaccination.
Please note it is parent’s/carer’s responsibility to inform the School Aged Immunisation Service if your child has had their immunisations elsewhere.
Please note this consent will last until your child leaves school. It is your responsibility to notify the school aged immunisation team if there are any changes to your child's medical history or if you change your mind about the vaccination. The school age immunisation team will continue to contact you annually whilst your child remains unvaccinated.
